Es una criatura hermafrodita que se adhiere y alimenta hasta llegar a la inevitable muerte de su víctima. La mayoría de las veces, dicho huésped, selecciona a su mártir desde una edad muy temprana, dando como resultado el desconocimiento de este sobre la existencia de aquel peso sobre su espalda, es decir, se desarrolla pensando que esta criatura es parte de él, ignorando por completo que se encuentra cargando su propia muerte.
A pesar de contar con dos corazones, este invertebrado es un ser despiadado que puede llegar a almacenar sangre hasta 5 veces el tamaño de su cuerpo, esto desde luego evita la muerte prematura de ambos, pues le permite a la víctima regenerar la sangre perdida mientras la sanguijuela entra en un estado de hibernación. Es curioso que algunas especies de sanguijuelas cuiden de su cría cargándola sobre su espalda, proporcionándole alimento, transporte, y protección, un comportamiento inusual para un invertebrado.
